Community Support
Community Support Services are designed to bridge the "knowledge" gap between identifying effective approaches to the delivery of mental health services and the "application" gap of putting those approaches into action in local communities. In order to deliver effective and time efficient Community Support Services to consumers, Turning Point Adolescent Center will work with consumers and agencies to develop a Person Centered Plan that establishes short and long-term goals individualized to reflect each consumer’s needs and strengths. The Person Centered Plan will also identify objectives and methods for achieving these goals. The plan addresses areas including but not limited to accessing home-based services, obtaining financial and medical benefits, securing psychiatric services, providing individual and family counseling, identifying effective crisis intervention plans, and recommending recreational opportunities. Community Support Services are available to any child or adult with a developmental or mental disability on a 24-hour basis.
